Opportunity Overview
A community-based program that provides crisis prevention and response services to individuals with intellectual and development disabilities and their families is seeking Coordinators (MSW, MHC, MPH, MPA, or MA/Psychology) to work with their program locations in Manhattan, Queens or the Bronx. Opportunities include Temp, Temp-to-Hire, and Direct Hire positions at $18-$20/hour depending on experience.
Responsibilities:
- Participate in didactic trainings and required trainings
- Provide systemic consultation
- Participate in National Center for Services trainings and events
- Prepare agendas and document outcomes for individual mentoring sessions held with the organization
- Maintain linkages and relationships with community partners
- Assist with referrals for consultation and treatment as needed
- Ensure the coordination of support meetings and crisis plan development for individuals served
- Participate in recurring meetings with leadership, clinical team, and Resource Center program
- Provide home visits and visits to day and vocational settings as needed
- Develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcomes plans for Resource Center admissions or inpatient hospital admissions and any other applicable documentations of services provided
- Maintain an active caseload of individuals referred for services
- Provide on-call support on a rotational basis
- Participate in the Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference as recommended
Requirements:
- MA in Mental Health, Psychology, Counseling, Social Work or other Human Service field and minimum of 2 yrs. Experience working with IDD/MI population
- Experience working with individuals with IDD and mental health and/or challenging behavior needs preferred
- One year supervisory experience
- Ability to become certified as a Coordinator within 12 months
- Bilingual Spanish a plus
